Diabetes Management Program

The goal of the Kelsey-Seybold Diabetes Management Program, a joint program of the KRF and KSC, is to prevent diabetic complications and subsequent hospitalizations through better diabetic control. Initiated in 1995, the program includes Certified Diabetic Educators, Diabetic Education Classes, a Diabetes Advisory Committee, monthly Diabetic reports to physicians, bi-annual Quality of Care reports to physicians, a diabetic education letter for patients, and a diabetic eye exam letter for patients. The Foundation tracks the results of critical diagnostic studies (e.g. LDL cholesterol, microalbumin, and blood sugar for diabetic patients), provides physicians with a monthly summary list for easy follow-up, and sends patients a reminder letter about the importance of diabetes education and appropriate medical care. The Kelsey Research Foundation also evaluates the effect of the Clinic’s Diabetes Education Programs on the control of diabetes in Kelsey-Seybold Clinic patients. This information is presented bi-annually to the Kelsey-Seybold Clinic Diabetes Advisory Committee to assist them in improving the care and health outcomes of diabetic patients.
